AS OF MONDAY 3RD JULY 2017 WE WELCOME OUR NEW NURSE PRACTITIONER TO THE TEAM.  PLEASE SEE THE APPOINTMENTS TAB FOR FURTHER INFORMATION.  Please note:The Doctors have instructed our Reception staff to ask all patients why they are coming in for an appointment in order to direct patients to the most appropriate healthcare professional.

Missed appointments during February, March & April 2017: 119 DOCTOR'S APPOINTMENTS AND 163 NURSES'S APPOINTMENTS that's a total of 282 appointments which were wasted due to people not turning up to booked appointments.

Local NHS changes to how we prescribe over the counter medications for short term illness and minor ailments: please ask your local pharmacist before making an appointment; buy over-the-counter medicines for minor problems and dont forget you can call NHS 111 for health advice 24 hours a day.

We have a new Patient Participation Group area in the waiting room. Please have a look at the information and fill in the form if you would like to join the group ..or you can submit the website form or speak to reception to show your interest the PPG. You will recieve details about meetings, talks and other news from your PPG.

To use Electronic Prescribing (EPS), you need to nominate your chosen Pharmacy to allow us to send your prescriptions electonically. You can nominate your pharmcy to us or via the pharmacy of your choice. Please speak to your local Pharmacist or speak to Reception.

Dont forget you can access your Summary Care Record, make and cancel appointments and order prescriptions using our online service. To register for this please bring photo ID into our reception.

New Patient Registration

 

This Practice has traditionally accepted new patients within the SS0 postcode area, which is our "Practice area".

Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as recent utility bills and ID documentation.

You will also need to complete a registration form (GMS1) and health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients must have a New Patient Check with our Healthcare Assistants to which you will need to bring a urine specimen.  Once you have had your New Patient Check you will be fully registered with the Practice.

Medical treatment is available from the date of full registration.
